Common symptoms
- persistent swelling of an arm, leg, trunk, breast or genital region;
- heaviness, tightness or aching;
- reduced movement or difficulty with clothing and shoes;
- skin thickening in later stages;
- recurrent cellulitis or infection.
Common causes
Lymphedema may occur after lymph node surgery, radiotherapy, cancer treatment, infection, trauma or congenital lymphatic problems. It can also coexist with venous disease, obesity, reduced mobility or lipedema.
Diagnosis
Assessment includes history, examination, limb pattern, skin changes, cancer treatment history, infection history and review of other causes of swelling. Imaging such as ICG lymphography, ultrasound or lymphoscintigraphy may be used in selected patients.
Treatment options
- compression garments or bandaging;
- complete decongestive therapy and manual lymphatic drainage;
- exercise, skin care and weight management where relevant;
- treatment of cellulitis and prevention of skin breakdown;
- selected surgery such as LVA / LVB, lymph node transfer or reductive procedures.
When to seek medical advice
Seek review for persistent swelling, sudden worsening, fever, spreading redness, pain, wounds, or recurrent infection. Early assessment is especially important after cancer treatment involving lymph nodes.
Frequently asked questions
Is lymphedema the same as water retention?
No. It is related to lymphatic drainage impairment, although other fluid-retention causes may coexist.
Can treatment cure it?
Most treatment improves control rather than providing a complete cure.
Who should assess it?
The right clinician depends on the cause. Persistent or complex swelling may need lymphedema therapy, vascular assessment, cancer follow-up or lymphatic surgery review.
